Dealing with injuries in children’s teeth is a little different than with grown adults; their incoming adult teeth have not fully come through and developed a strong, established root.

The apex, or tip of the root (way down/up in the gums), in a tooth that has not yet matured in a child’s mouth, has a far larger opening than a mature tooth. The reason why this opening is wider during the young stages is so that nutrients can more easily enter the tooth to help it develop strength and health as the child grows up. Compare it to an ice cream waffle cone, where the crown of the tooth is where the scoop of ice cream goes, and the apex is the pointy part at the bottom. If you were to bite a hole in the bottom of the cone, it would leave an opening… and ice cream would drip out.

Well, the anatomy of a tooth is similar. In a still-developing adult tooth, the hole is just much wider, and the walls of the root canal are thinner. As the child and his teeth mature, the apex opening grows smaller, the walls thicker, and the tooth becomes nice and sturdy.

If an injury were to occur to these young adult teeth, special care is needed if the health of the tooth is to be preserved to its full potential. One of two kinds of procedures may be needed: apexogenesis or apexification. I will cover these in future posts.

Implants are used to replace teeth that have been lost or extracted, possibly due to disease or injury. A new “root” is drilled directly through the gums into the upper or lower jaw bone (depending on where the implant is needed) and is secured there. This new “root” will serve as the anchor for an artificial replacement tooth.

As usual, it is important to keep the area sterilized while surgery is being performed. Dental implant surgery goes deeper, literally, than each individual tooth. After surgery, there is a healing period where the surrounding bone and gums must be monitored to ensure that the new teeth are properly integrated into healthy oral function (this includes avoiding gum recession, for example, or infections in the mandible). Ozone is wonderful because it sterilizes more effectively and safely than traditional chemical sterilizers; not only this, but it also stimulates healthy gum healing and helps infuse the bone structure with strengthening minerals.

As its name implies, periodontal review is examination of the health of your gums.

The first portion of the exam involves periodontal measurements which are readings, taken at the gumline with a hand probe, around each tooth. Measurements are taken millimeters. Healthy gums have a measurement of 0-3 mm and diseased gums are indicated by measurements of 4mm and greater.

Next we examine the gum tissue’s color and texture. Healthy gums are firm and do not bleed. Gum tissue responds rapidly to disease and when disease is present, they will change in color appearance and may begin to bleed.

We also check bone height. The bone supports the tooth and is very susceptible to bacterial infection and trauma from clenching and grinding.

Finally, we examine the esthetic component and look at how the gums line up in relationship to the smile line.
